Intralipid IV: A Promising Option for Recurrent Pregnancy Loss

By Dr. Jennifer Kerr, ND7/23/2025

Fertility struggles are quickly becoming a major concern for patients and doctors alike, with Estimates suggesting that approximately one in every six people of reproductive age worldwide experience infertility in their lifetime1.

Recurrent pregnancy loss (RPL) is a deeply distressing experience for many couples, often leaving them searching for answers and hope. While traditional investigations may not always reveal a clear cause, recent advances in reproductive immunology have opened new doors—including the use of intralipid IV therapy. Let’s explore why intralipid IV is gaining attention as a positive, innovative approach for those facing RPL.

What Is Intralipid IV Therapy?

Intralipid is an intravenous fat emulsion, originally developed for nutritional support, that has found a new role in fertility clinics. Its unique ability to modulate immune function in the uterus is at the heart of its use in reproductive medicine2.

Why Consider Intralipid IV for RPL?

  • Targeting Immune Imbalances:
Many cases of RPL are believed to be linked to immune system dysfunction, particularly overactive uterine natural killer (uNK) cells. Intralipid IV therapy is thought to help by calming this immune response, creating a more welcoming environment for embryo implantation.
  • Encouraging Clinical Evidence:
Several meta-analyses and clinical studies have shown that intralipid IV can significantly improve pregnancy outcomes for women with RPL, especially when immune factors are suspected. Highlights from the research include:
    • Higher clinical pregnancy rates
    • Increased ongoing pregnancy and live birth rates
    • Reduced miscarriage rates in select patient groups
  • Safety Profile:
Intralipid infusions are generally well-tolerated, with minimal side effects reported in the literature. This makes it a promising option for women who have exhausted other avenues

Who Might Benefit?

Intralipid IV is not for everyone undergoing fertility treatment. It is most promising for:

  • Women with a history of recurrent pregnancy loss where immune factors are suspected
  • Patients who have not responded to conventional treatments
  • Those who have undergone immune testing and have evidence of elevated uNK cell activity or other immune markers

What to Expect During Treatment

  • Administration:
The infusion is given intravenously, usually before embryo transfer and sometimes repeated in early pregnancy.
  • Monitoring:
Patients are monitored for any allergic reactions, though these are rare.

Do I Need To Bring Anything To The Appointment?

  • Recent blood work:
If you have blood work that was done in the past 6 months that includes liver, kidney, cholesterol, and blood sugar markers, that is crucial to assess to make sure you are a good candidate for intralipid IV therapy.
  • Immune markers:
If you have blood work showing natural killer cells or other immune markers that is also helpful for the doctors to analyze3.

A Message of Hope

For couples struggling with the heartbreak of recurrent pregnancy loss, intralipid IV therapy offers a beacon of hope. While more research is needed to refine patient selection and protocols, the current evidence is encouraging—especially for those with immune-related causes of RPL. If you are navigating this challenging journey, talk to your fertility specialist about whether intralipid IV might be a suitable option for you.
